Important book of halacha and fear of Heaven. "Accurately proofread by… R. Yehuda Ya'ale (acronym) of Pesaro Zvi (acronym)".
R. Eleazar of Worms (1165-1240), son and disciple of R. Yehudah son of Rabbi Klonymus of Mainz and disciple of R. Yehudah HaChassid, author of Sefer Chassidim, who instructed him in the study of Kabbalah. Composed approximately thirty books, and is known by the name of this book, Ba'al HaRokeach.
Signatures on first leaf: "…Yosef Z---?", "Avraham Reuven", "Ya'akov…". The first leaf and additional leaves are stamped "Beit Midrash Menachem Zion in the Churva of R. Y. HeChassid in Jerusalem" and bear handwritten inscriptions of R. Zundel Salant: "L'Midrash Menachem Zion".
