Piskei Halachot by the Kabbalist R. Menachem of Recanati. Bologna: "HaShutafim", [1538]. First edition.
The author, R. Menachem ben Binyamin Recanati (1250-1310), was one of the first Italian kabbalists. This is his book of halachic rulings, which became an important source of rulings and is quoted by many Torah authorities such as the Beit Yosef, the Shach and the Magen Avraham.
Many stamps of R. Moshe Chalfan Lifshitz (died Cheshvan 1830), one of the leaders of the Warsaw Jewish community. He was the father-in-law of the first Rebbe of Gur, R. Yitzchak Meir Alter, author of "Chidushei HaRim" and of Rebbe Menachem Mendel of Kotzk. [The book "Siach Sarfei Kodesh" contains many stories about R. Moshe's great holiness]. He was a disciple of the Maggid of Kozhnitz and the Chozeh of Lublin, both of whom were involved in arranging a match with his son-in-law the Chidushei HaRim.
