Sefer Chessed Le-Avraham, on Kabbalah issues, by Rabbi Avraham Azulay of Hebron. Slavita, 1794. Of the first books that were printed by Rabbi Moshe Shapira.
Approbation by Rabbi Ya'akov Shimshon of Shpitovka.
[According to this approbation in The Bibliography Institute CD record 0108497 printing is referred to Rabbi Pinchas of Koritz father of Rabbi Moshe Shapira, but this seems impossible since Rabbi Pinchas of Koritz died in 1790, while on the title page the date reads 1784. This approbation might have been written while Rabbi Pinchas of Koritz was still alive and the books were printed by his son Rabbi Moshe after his death].
An additional approbation by Rabbi Aryeh Leib Ben Shalom Ha-Levy of Zbaraz, Av-Beit-Din of Voltchisak, "to the printers of Slavita". [It is worthwhile noting that Rabbi Yossef Segal Av-Beit-Din of Yassi, is mentioned on title of Or Hachaim printed in Slavita in those days].
Signatures and ownership handwritten inscriptions in ancient handwriting: "Nachum… Avraham Menachem Mendel".
